Royal Bank of Scotland came on third with 19,214 complaints and HSBC fourth with 16,573.
The FOS saw a 110% increase in new complaints in this period, receiving a record total of 283,251 new cases.
Complaints about PPI made up 74% of the total complaints referred to the FOS between 1 July and 31 December 2012.
